Form I-693 Validity Policy

USCIS has updated its policy regarding Form I-693, the Report of Immigration Medical Examination and Vaccination Record, for forms signed by a civil surgeon on or after November 1, 2023. Effective June 11, 2025, a Form I-693 is valid only while the associated immigration application—such as Form I-485 (Application to Register Permanent Residence or Adjust Status)—is pending. If that application is withdrawn or denied, the Form I-693 becomes invalid. As a result, applicants who refile Form I-485 after a denial or withdrawal must submit a newly completed Form I-693.

This update reverses a previous policy issued on April 4, 2024, which allowed indefinite validity for Forms I-693 signed on or after November 1, 2023. USCIS has now determined that the earlier guidance was overly broad and posed potential public health risks by permitting outdated medical exams for future applications. By restricting the validity of Form I-693 to the current pending application, USCIS aims to ensure timely and accurate medical evaluations, thereby supporting public health protection efforts.
